Providing a safe learning environment for our students is key. In order to accomplish this, we need your help to provide our staff with medical information that would best equip our staff to respond to meet your child's needs.
Here are some emergency health plan templates available to have completed by your child's primary health care provider:

**If your child has a health condition that is not listed above, or your provider has a different form for the health condition listed, the school district will accept health plans that are created by your child's primary health care provider.**

Screenings
Vision Screening
Vision can impact learning, and many times children are unaware of signs of the vision problems. This is why early detection is key. To help our students be as successful as they can be, we have partnered with the Buttes des Morts Lions Club to offer vision screening with parent/guardian consent, to students in elementary and any student with recommendations from parent/guardian, student or teacher.
**Screening is a tool to help determine if your child requires a referral to a vision care specialist. It does not detect all eye diseases and/or conditions.
Hearing Screenings
The effects of hearing loss on children's communication, academic achievement, social interactions, and self-concept can be significant. Early detection through school hearing programs is crucial for timely intervention to minimize the negative impacts on speech, language skills, learning, behavior and future educational choices. To help our students succeed, our district conducts annual hearing screening for kindergarten students with parent/guardian consent and when requested by parent/guardian, student or teacher.
**Screening is a tool to help determine if your child requires a further evaluation by a pediatrician or hearing specialist. It does not detect all diseases and/or conditions.
Scoliosis Screenings
Scoliosis screenings are completed when requested by the parent/guardian or recommended by a teacher with parent/guardian consent.
Dental Services
The Winneconne Community School District has continued our partnership with Bridging Brighter Smiles, a dental program that offers oral hygiene services such as oral screenings, cleanings, fluoride varnish and dental sealants, to students with Forward Health (BadgerCare) or no insurance at a reasonable to low cost. The best part is, they come right to our school premises during the school day to provide these services to our students 4K through 12th grade!
